Extended-spectrum beta-lactamase-producing organisms resistant to third-generation cephalosporins have been increasingly implicated in nosocomial infection.'Extended-spectrum beta-lactamase-producing Klebsiella pneumoniae (ESKP) has been endemic at Tygerberg Hospital since 1993, and has recently been implicated in an outbreak of sepsis in the neonatal unit.' Despite improved infection control practices, sporadic episodes of nosocomial sepsis due to ESKP still occur both in the neonatal unit and in other paediatric wards. To the best of our knowledge this is the first report of a child with endocarditis due to ESKP.
