Exploring registered nurses' understanding of the concept of critical thinking in a private hospital in the Gauteng Province, South Africa

Abstract
ENGLISH SUMMARY: Background: In hospital environments, registered nurses are faced with difficult scenarios needing appropriate critical thinking skills. As health care systems become more sophisticated, these critical thinking abilities help registered nurses to be able to synthesise information and make swift, appropriate clinical decisions that lead to safe and efficient health care delivery. The current study was based on the researcher’s concern regarding an apparent lack of disposition towards critical thinking amongst registered nurses working in medical and surgical wards. Registered nurses are essential healthcare professionals in meeting patient needs in a health care system. Method: This study aimed to explore and describe registered nurses’ understanding of critical thinking in their clinical practice in medical and surgical wards in a private hospital in South Africa. The objective of this study was to explore how registered nurses understand critical thinking as a concept in their clinical practice. The researcher used a qualitative approach for this study. The approach applied an exploratory, descriptive design to explore the understanding and description of the concept of critical thinking by registered nurses who are working in medical and surgical wards. Data was analysed using a thematic analysis approach, which entailed a process of reading and re-reading data, coding, and generating themes from which data was interpreted and findings deduced. To conduct this research study participants were drawn from the group of registered nurses working in the medical and surgical wards of the study site who met the inclusion criteria. The final sample size comprised twelve registered nurses (RNs). Semi-structured individual interviews supported by an interview guide were used to gather data from participants. Results: The findings indicated a lack of understanding of the concept of critical thinking by registered nurses. Even though some nurses had an idea of the concept, it appeared that there is a big gap in nursing education and clinical teaching when facilitating achieving critical thinking as one of the objectives. The themes that emerged from the data indicated that there are factors that have a negative influence on how registered nurses think and apply their minds in clinical wards. Conclusion: The study concludes that registered nurses need support in developing critical thinking skills. It was also deduced that some demarcations and limited resources impede critical thinking in the wards.
AFRIKAANSE OPSOMMING: Agtergrond: Geregistreerde verpleegkundiges word in hospitaalomgewings gekonfronteer met moeilike scenario’s wat toepaslike kritiese denkvaardighede benodig. Namate gesondheidsorgstelsels meer gesofistikeerd raak, help hierdie kritiese denkvermoe geregistreerde verpleegkundiges om inligting te sintetiseer en vinnige, toepaslike kliniese besluite te neem wat tot veilige en doeltreffende gesondheidsorglewering lei. Die huidige studie was gebaseer op die navorser se kommer oor ’n oenskynlike gebrek aan ingesteldheid teenoor kritiese denke onder geregistreerde verpleegkundiges wat in mediese en chirurgiese sale werk. Geregistreerde verpleegkundiges is noodsaaklike gesondheidswerkers om aan pasientbehoeftes in ‘n gesondheidsorgstelsel te voldoen. Metode: Hierdie studie het ten doel gehad om geregistreerde verpleegkundiges se begrip van kritiese denke in hul kliniese praktyk in mediese en chirurgiese sale in ’n privaat hospitaal in Suid-Afrika te verken en te beskryf. Die doel van hierdie studie was om ondersoek in te stel hoe geregistreerde verpleegkundiges kritiese denke as ’n konsep in hul kliniese praktyk verstaan. Die navorser het ’n kwalitatiewe benadering vir hierdie studie gebruik. Die benadering pas ’n verkennende, beskrywende ontwerp toe om die begrip en beskrywing van die konsep van kritiese denke deur geregistreerde verpleegkundiges wat in mediese en chirurgiese sale werk, te ondersoek. Data is ontleed deur gebruik te maak van ’n tematiese ontledingsbenadering, wat ’n proses is van lees en herlees van data, kodering en generering van temas waaruit data geinterpreteer en bevindinge afgelei kan word. Hierdie navorsingstudie het bestaan uit deelnemers uit die groep geregistreerde verpleegkundiges wat in die mediese en chirurgiese afdelings van die studieterrein werksaam is en wat aan die insluitingskriteria voldoen het. Die finale steekproefgrootte het uit twaalf geregistreerde verpleegkundiges (RN'e) bestaan. Semi-gestruktureerde individuele onderhoude ondersteun deur ’n onderhoudsgids is gebruik om data van deelnemers in te samel. Resultate: Die bevindinge het gedui op geregistreerde verpleegkundiges se gebrek aan begrip van die konsep van kritiese denke. Alhoewel sommige verpleegkundiges ’n idee van die konsep gehad het, het dit geblyk dat daar ’n groot leemte in verpleegopleiding en kliniese onderrig is wanneer die bereiking van kritiese denke as een van die doelwitte gefasiliteer word. Die temas wat uit die data na vore gekom het, het aangedui dat daar faktore is wat ’n negatiewe invloed het op hoe geregistreerde verpleegkundiges dink en hul denke in kliniese sale toepas. Slotsom: Die studie kom tot die slotsom dat geregistreerde verpleegkundiges ondersteuning benodig in die ontwikkeling van kritiese denkvaardighede. Daar is ook afgelei dat sommige afbakenings en beperkte hulpbronne kritiese denke in die sale belemmer.
