Exploring the barriers to training and development of medical doctors in the public health system : a case for Free State Academic Hospitals

Abstract
ENGLISH SUMMARY: The study explores barriers to the training and development of medical doctors in the public healthcare system using Free State academic hospitals, Pelonomi tertiary and Universitas central, as reference. Human resources form part of the public healthcare system pillars because of the labour intensity of the system. It is natural that, among others, the cadre of medical doctors, as part of human resources, is essential for the survival of the public healthcare system. The rationale for the study emanates from the necessity to improve the training and development of medical doctors in the public healthcare domain and to develop literature in the realm of training and development for medical doctors in the public healthcare system. The literature review captures theories, definitions and divergent views of scholars and academics on the training and development of medical doctors in the public healthcare system. The legislative framework highlights the importance of legislation that has an impact on the training and development of medical doctors in the public healthcare system. The structure and mandate of the Free State Department of Health are explained in detail. The research methodology and design of the study include both mixed method research. After satisfying the ethical requirements, four senior managers of the Free State Health Department were interviewed and thirty-three medical resident doctors – of a target population of forty-eight – of Pelonomi tertiary and Universitas central hospitals completed a questionnaire. The interview participation and questionnaire completion were 100 per cent and 69 per cent respectively. The study revealed that barriers to the training and development of medical doctors in the public healthcare system, with reference to Free State academic hospitals, do exist. These barriers relate to the following: a lack of capacity for the training and development of the Free State Department of Health; a weak partnership between the Free State Department of Health, the University of the Free State medical school and other similar public healthcare facilities (academic hospitals); limited time due to high patient volumes; a shortage of staff; overtime calls per medical doctor; limited resources for training and development; a shortage of consumables and medical equipment, and COVID-19 has worsened the challenges. The study concludes with recommendations that are aimed at addressing the barriers and challenges revealed by the study. The study recommends the following: training and development of the Free State Department of Health must be capacitated; the Free State Department of Health’s partnership with the Free State University medical school, and other similar public health facilities (academic hospitals), must be intensified and all other identified challenges must be addressed as recommended in the final chapter of the study.
AFRIKAANSE OPSOMMING: Die studie ondersoek struikelblokke tot die opleiding en ontwikkeling van mediese dokters in die openbare gesondheidsorgstelsel deur Vrystaatse akademiese hospitale, Pelonomi tersiere en Universitas sentraal, as verwysing te gebruik. Menslike hulpbronne vorm deel van die pilare van die openbare gesondheidsorgstelsel vanwee die arbeidsintensiteit van die stelsel. Dit is natuurlik dat, onder andere, die kader van mediese dokters, as deel van menslike hulpbronne, noodsaaklik is vir die voortbestaan van die openbare gesondheidsorgstelsel. Die rasionaal vir die studie spruit uit die noodsaaklikheid om die opleiding en ontwikkeling van mediese dokters in die openbare gesondheidsorgdomein te verbeter en om literatuur op die gebied van opleiding en ontwikkeling vir mediese dokters in die openbare gesondheidsorgstelsel te ontwikkel. Die literatuuroorsig bevat teoriee, definisies en uiteenlopende sienings van geleerdes en akademici oor die opleiding en ontwikkeling van mediese dokters in die openbare gesondheidsorgstelsel. Die wetgewende raamwerk beklemtoon die belangrikheid van wetgewing wat 'n impak het op die opleiding en ontwikkeling van mediese dokters in die openbare gesondheidsorgstelsel. Die struktuur en mandaat van die Vrystaatse Departement van Gesondheid word breedvoerig verduidelik. Die navorsingsmetodologie en ontwerp van die studie sluit beide kwalitatiewe en kwantitatiewe metodes in. Nadat aan die etiese vereistes voldoen is, is onderhoude gevoer met vier senior bestuurders van die Vrystaatse Gesondheidsdepartement en drie-en-dertig mediese inwonende dokters – van ’n teikenpopulasie van agt-en-veertig – van Pelonomi-tersiere en Universitas sentrale hospitale het ’n vraelys voltooi. Die onderhoudsdeelname en vraelysvoltooiing was 100 persent en 69 persent onderskeidelik. Die studie het aan die lig gebring dat struikelblokke tot die opleiding en ontwikkeling van mediese dokters in die openbare gesondheidsorgstelsel, met verwysing na Vrystaatse akademiese hospitale, wel bestaan. Hierdie hindernisse hou verband met die volgende: 'n gebrek aan kapasiteit vir die opleiding en ontwikkeling van die Vrystaatse Departement van Gesondheid; ’n swak vennootskap tussen die Vrystaatse Departement van Gesondheid, die Universiteit van die Vrystaatse mediese skool en ander soortgelyke openbare gesondheidsorgfasiliteite (akademiese hospitale); beperkte tyd as gevolg van hoe pasientvolumes; 'n tekort aan personeel; oortydoproepe per mediese dokter; beperkte hulpbronne vir opleiding en ontwikkeling; 'n tekort aan verbruiksgoedere en mediese toerusting, en COVID-19 het die uitdagings vererger. Die studie sluit af met aanbevelings wat daarop gemik is om die struikelblokke en uitdagings wat deur die studie aan die lig gebring is, aan te spreek. Die studie beveel die volgende aan: opleiding en ontwikkeling van die Vrystaatse departement van gesondheid moet bekwaam wees; die Vrystaatse Departement van Gesondheid se vennootskap met die Vrystaatse Universiteit se mediese skool, en ander soortgelyke openbare gesondheidsfasiliteite (akademiese hospitale), moet verskerp word en alle ander geidentifiseerde uitdagings moet aangespreek word soos aanbeveel in die laaste hoofstuk van die studie.
