Dept. of Industrial Engineering.The past 50 years have seen the world-wide establishment of large and centralised facilities providing clinical laboratory services at a relatively low cost per test to the health care market. In South Africa there has been the amalgamation of three independent pathology groups (Drs. du Buisson & Partners, Drs. Bruïnette, Kramer & Partners and Drs. Bouwer & Partners) to utilise their combined volumes and regional networks to achieve economies of scale and scope. These pathology laboratories face the opposing challenges of increased expectations from professionals using the laboratory and the drive from the funders of health care that demand a fast and accurate service at a lower cost. The advantages of value adding by performing routine testing at the peripheral sites are declining as the cost of providing this service increases. Through the re-engineering of the organisation and the integration of the physical and virtual value chain it is becoming increasingly possible to centralise the routine pathology tests in order to contain the cost of producing these results.
