dc.description.abstractENGLISH ABSTRACT: The aim of the study was to design a treatment program for burn patients who abuse alcohol. To achieve this a literature study was done on the psycho-social support of burn patients and on substance abuse counselling. An empirical study was designed to determine the prevalence of alcohol dependence in patients at the burns unit of the Tygerberg Hospital and to describe patterns of alcohol consumption, socio-economic adjustment and attitudes towards drinking. The subjects of the study were patients over the age of eighteen years who were admitted consecutively to the burns unit between 21 March 1992 and 19 June 1992. During this period 79 patients were admitted. Twenty-six were excluded from the study because of early discharge (eleven patients), language problems (five patients) and deaths (ten patients). The remaining 53 patients were screened for alcohol problems using the Michigan Alcoholism Screening Test (Selzer 1971). Thirty patients screened positively for alcoholism. A more comprehensive interview schedule was administered to these patients to obtain additional information. Questionnaires were utilized to obtain information on the existing treatment facilities for alcohol dependent clients in the Western Cape. The most important results from this study were as follows. The incidence of alcohol dependence in the study group was high. Most patients indicated that they were regular weekend drinkers. More than half of the patients reported previous alcohol-related accidents and many patients attributed their present burn injury to alcohol intoxication. The study group can be described as socio-economically deprived. It was also found that the burn injury represents a crisis during which treatment for alcohol problems can be introduced. The proposed treatment program is based on the literature study and on the findings of the empirical study. Individual couselling and groupwork is combined in an integrated program. The program represents a minimal intervention strategy aimed at attitude and behaviour change.en_ZA
dc.description.abstractAFRIKAANSE OPSOMMING: Die doel van die studie was om 'n hulpverleningsprogram te skryf vir brandwondpasiënte wat alkohol misbruik. Ten einde die hulpverleningsprogram te skryf, is 'n literatuurstudie gedoen oor die psigososiale ondersteuning van die brandwondpasiënt en die hulpverlening aan die alkoholafhanklike persoon. 'n Empiriese ondersoek is ook gedoen om die insidensie van alkoholmisbruik by brandwondpasiënte van die Tygerberg-hospitaal te bepaal en om die aard van die pasiënte se alkoholverwante probleme, hulle maatskaplike omstandighede en bestaande hulpverleningsprogramme te beskryf. Die steekproef is verkry deur alle volwasse pasiënte wat agtereenvolgens vanaf 21 Maart 1992 tot 19 Junie 1992 tot die brandwondeenheid toegelaat is, by die studie te betrek. Gedurende hierdie tydperk is 79 volwasse pasiënte tot die eenheid toegelaat. Ses en twintig pasiënte is uitgesluit van die studie weens vroeë ontslag (elf pasiënte), taalprobleme (vyf pasiënte) en sterftes (tien pasiënte). Op hierdie wyse is 53 proefpersone verkry. Daar is hoofsaaklik gebruik gemaak van gestruktureerde onderhoude om inligting in te samel. Die eerste deel van die onderhoudskedule is met al 53 proefpersone behandel. Dit het bestaan uit identifiserende inligting en 'n vraelys wat ontwerp is om alkoholafhanklikheid te identifiseer. Vir hierdie doel is gebruik gemaak van die Michigan Alcoholism Screening Test (Selzer 1971). Alkoholafhanklikheid is sodoende by 30 van die pasiënte geïdentifiseer. Die res van die onderhoudskedule het gehandel oor die ciard van die pasiente se alkoholverwante probleme, hulle houding teenoor afhanklikheid en hulpverlening en oor hulle maatskaplike omstandighede en is met hierdie 30 pasiënte behandel. lnligting oor bestaande hulpverleningsprogramme is bekom deur vraelyste aan die spesialiteitsorganisasies en inrigtings in die WesKaap wat hulpverlenings-programme vir afhanklike persone aanbied, te stuur. Die belangrikste bevindinge was dat daar 'n hoë insidensie van alkoholafhanklikheid by brandwondpasiënte is, dat pasiënte hoofsaaklik oor naweke alkohol misbruik, dat baie van die pasiënte vorige alkoholverwante beserings opgedoen het dat 'n groot persentasie van die pasiënte hulle huidige brandwondbesering toeskryf aan die feit dat hulle onder die invloed van alkohol was toe hulle die besering opgedoen het, dat die pasiënte uit swak maatskaplike omstandighede kom en dat die brandwondbesering vir die pasiënte 'n krisissituasie skep, waartydens hulpverlening vir alkoholafhanklikheid geïnisieer kan word. Die hulpverleningsprogram wat geskryf is, is gebaseer op die literatuurstudie en bevindinge wat uit die empiriese ondersoek gemaak is. Die program kan beskou word as 'n minimale ingrypingstrategie waarin individuele hulp en groepwerk op 'n geïntegreerde wyse benut word om die pasiënt te help om sy gedrag positief te verander.af_ZA
