Gauteng medical orthotist prosthetists perceptions about self-screening to detect adolescent idiopathic scoliosis : a qualitative exploration

Abstract
ENGLISH SUMMARY: Introduction: Early diagnosis of adolescent idiopathic scoliosis is the key to successful conservative treatment with a corrective spinal orthosis. South Africa has no official adolescent idiopathic scoliosis screening programmes. Aim of the study: The purpose of the study was to explore the perceptions of Gauteng medical orthotist prosthetists with regard to self-screening for the detection of adolescent idiopathic scoliosis. Methods: A qualitative descriptive design was used. The study was done in Gauteng, South Africa with a sample of seven purposefully selected medical orthotists and prosthetists. Data were collected through semi-structured interviews. Thematic analysis was used to identify emerging themes. Findings: Four themes emerged that were identified. The first theme, past and present adolescent idiopathic scoliosis detection experiences describes the advantages of early detection and corrective spinal orthosis management versus poorer health outcomes and the need for surgery if detection is delayed. The second theme, attitudes towards introducing adolescent idiopathic scoliosis self-screening highlighted participants positive perspectives regarding implementing adolescent idiopathic scoliosis self-screening. The third theme, suggestions for implementing adolescent idiopathic scoliosis self-screening presents practical implementation strategies including the use of printed and electronic media, the importance of cultural sensitivity, and schools as a probable point of dissemination. The fourth theme, possible barriers to adolescent idiopathic scoliosis self- screening describes possible logistical, technological, economic, and attitudinal barriers. Conclusion: Participants believed an adolescent idiopathic scoliosis self-screening programme needs to be implemented in South Africa. They suggested that this programme should be acceptable, accessible, affordable, and understandable to children and parents from all cultural groups. They also recommended that the programme be designed to empower children and parents to recognise spinal deviation and guide them towards professional assistance.
AFRIKAANSE OPSOMMING: Doel van die studie: Die doel van die studie was om die persepsies van Gautengse geneeskundige ortotis prototiste met betrekking tot selfondersoek vir die opsporing van adolessente idiopatiese skoliose te ondersoek. Metodes: 'n Kwalitatiewe beskrywende navorsingsontwerp is gebruik. Die studie is in Gauteng, Suid-Afrika, gedoen met 'n steekproef van sewe geneeskundige ortotis prototiste wat doelgerig gekies. Data is versamel deur middel van semi-gestruktureerde onderhoude en tematiese ontleding is gebruik om data te ontleed. Bevindinge: Vier temas is geidentifiseer. Die eerste tema, ervarings van die verlede en huidige adolessente idiopatiese skoliose-opsporing, beskryf die voordele van vroee opsporing en regstellende hantering van spinale ortose teenoor swakker gesondheidsresultate en die behoefte aan chirurgie as die opsporing vertraag word. Die tweede tema, houdings ten opsigte van die instelling van adolessente idiopatiese skoliose selfondersoek, het deelnemers positiewe perspektiewe op die implementering van adolessente idiopatiese skoliose selfondersoek uitgelig. Die derde tema, voorstelle vir die implementering van selfondersoek van adolessente idiopatiese skoliose, bied praktiese implementeringstrategiee, waaronder die gebruik van gedrukte en elektroniese media, die belangrikheid van kulturele sensitiwiteit en skole as 'n waarskynlike punt van verspreiding. Die vierde tema, moontlike hindernisse vir adolessente idiopatiese skoliose selfondersoek, beskryf moontlike logistieke, tegnologiese, ekonomiese en houdingsversperrings. Gevolgtrekking: Deelnemers was van mening dat 'n adolessente idiopatiese skoliose-selfondersoekprogram in Suid-Afrika geimplementeer moet word. Die program moet aanvaarbaar, toeganklik, bekostigbaar en verstaanbaar wees vir kinders en ouers uit alle kulturele groepe. Die program moet kinders en ouers in staat stel om ruggraatafwyking te herken en hulle te lei tot professionele hulp.
