Development and testing of a “zero stunting” scorecard at provincial level : Gauteng, South Africa

Abstract
ENGLISH SUMMARY: Introduction: Commitment and capacity at different levels are required to ensure that governments reach the international target of reducing stunting by 40 percent by 2025. Aim: To develop and test a “Zero Stunting” scorecard for use by government at sub-national/provincial level in Gauteng, South Africa. Methods: A draft “Zero Stunting” scorecard prototype was developed and tested for usability and value in the Western Cape Province. The current study followed the same methodology and utilized the same data collection tools. The draft scorecard was developed, grounded in the latest available evidence on the context, causes and consequences of stunting. It is also based on the premise of the Hunger and Nutrition Commitment Index (HANCI), which consist of three domains: law, policy and finance. On the scorecard, proposed indicators are grouped under the themes of food control, political economy, food systems, education, nutrition, water, sanitation and environment, health, social protection, finance and governance. The draft scorecard was used as a tool during semi-structured interviews with Gauteng government officials to identify the potential value and usability thereof and to establish availability and relevance of provincial data that can be used to measure the proposed indicators. Results: Gauteng government officials had a vague understanding of the meaning of stunting, but the long term impact on human capital was not known. The participants were of the opinion that the “Zero Stunting” scorecard could be of value and use to hold departments accountable with regards to 1) improving monitoring and evaluation, 2) taking ownership and evidence informed decisions, 3) making better use of the available provincial data through identifying the gaps in services, with emphasis on health coverage and by reaching the poorest population groups and 4) thereby ensuring co-ordinated interventions. Data to populate the scorecard was mostly available at provincial level. Participants felt that a central unit should be made responsible for co-ordinating the actions required for stunting reduction. The office of the Premier was mentioned as the most appropriate department to fulfil this function. Conclusion: The proposed “Zero Stunting” scorecard, holds potential to be used as a valuable data visualisation tool to track progress on stunting reduction at provincial level in Gauteng.
AFRIKAANSE OPSOMMING: Inleiding: Toewyding en kapasiteit op verskillende vlakke is nodig om te verseker dat regerings die internationale teiken bereik om “dwerggroei” (stunting) teen 2025 met 40 persent te verminder. Doelwit: Die doel van die studie was om ‘n “Zero Stunting” telkaart te ontwikkel en te toets vir die gebruik van regerings op sub-nasionale / provinsiale vlak. Metodes: ‘n Konsep-prototipe “Zero Stunting” telkaart is ontwikkel en getoets vir bruikbaarheid en waarde in die Wes-Kaap Provinsie. Die huidige studie het dieselfde metodiek gevolg en dieselfde data-insamelingsvorms gebruik. Die telkaart is ontwikkel gegrond op die nuutste beskikbare wetenskappe bewyse vir die konteks, oorsake en gevolge van dwerggroei. Dit is ook gebaseer op die “Hunger and Nutrition Commitment Index” (HANCI), wat bestaan uit drie domeine: wetgewing, beleid en finansies. Die konsep “Zero Stunting” telkaart het voorgestelde indikatore gegroepeer onder die temas: voedselsisteme, onderwys, voeding, water, sanitasie en omgewing, gesondheid, sosiale beskerming, finansies en bestuur. Die telkaart is daarna gebruik as hulpmiddel tydens semi-gestruktureerde onderhoude met regeringsamptenare in Gauteng om die potensiele waarde en bruikbaarheid daarvan te identifiseer en om beskikbaarheid en relevansie vas te stel van provinsiale data wat gebruik kan word om die voorgestelde indikatore te meet. Resultate: Gauteng regeringsamptenare het ‘n vae begrip gehad van die betekenis van dwerggroei, maar die langtermyn-impak op mensekapitaal was nie bekend nie. Die deelnemers was van mening dat die “Zero Stunting” telkaart van waarde en nut kan wees om departemente aanspreeklik te hou met betrekking tot die verbetering van monitoring en evaluering, 2) besluite oor eienaarskap en bewyse, 3) betere gebruik van die beskikbare provinsiale gegewens deur die identifisering van die leemtes in dienste, met die klem op gesondheidsdekking en deur die armste bevolkingsgroepe te bereik en 4) om sodoende gekoordineerde ingrypings te verseker. Data om die telkaart in te vul, was meestal op provinsiale vlak beskikbaar. Deelnemers was van mening dat ‘n sentrale eenheid verantwoordelik moet wees om die aksies te koordineer wat nodig is vir die vermindering van dwerggroei. Die kantoor van die Premier is genoem as die mees geskikte departement om hierdie funksie te vervul. Gevolgtrekking: Die voorgestelde “Zero Stunting” –telkaart het die potensiaal om gebruik te word as ‘n waardevolle instrument vir die visualisering van data om vordering met die vermindering van dwerggroei op provinsiale vlak in Gauteng op te volg.
