Role modelling : a critical component of bedside teaching in a critical care unit

Abstract
ENGLISH SUMMARY: In the evolving and complex healthcare system, clinical teaching and training of new nurses in the critical care unit is indispensable to ensure safe and quality healthcare. The registered professional nurses’ (RPN) awareness and understanding of role modelling being a crucial teaching and learning method depends on their contribution to growth and development. Role modelling is a pervasive means where development through observation and practice in the clinical environment occur. Therefore, it is a foundation in helath professions education. The aim of the study is to determine how RPNs understand role modelling as a teaching strategy. A qualitative, phenomenological design was used to obtain data by conducting semi-structured interviews with purposive sampling of seven RPNs working in the critical care unit of a tertiary teaching hospital in the Western Cape. Interviews were audio recorded and transcribed verbatim. The transcribed data were analysed and manually coded, which generated two themes that illustrated the findings of the study. The first theme illustrated the participants’ understanding of role modelling and revealed that new nurses intentionally and unintentionally aspire to imitate specific behaviour and that they aquire knowledge and skills in critical care nursing, including soft skills. Furthermore, reflection was used to link information or actions to foster learning. The second theme found that a number of challenges prevent RPNs and new nurses from teaching and learning through role modelling. The findings illustrated that the RPNs do not understand that role modelling is implicit in executing their daily tasks. Participants’ eagerness to use rolemodelling as a teaching and learningmethod could not be questioned, but their utilisation thereof should be reinforced. The study represents the first of its kind at this institution and presumes that this contribution could add value to the understanding of role modelling as a teaching method for healthcare practitioners.
AFRIKAANSE OPSOMMING: In die ontwikkelende and komplekse gesondheidsorgsisteem, is die kliniese onderrig en opleiding van nuwe verpleegsters in die kritieke sorgeenheid onmisbaar om veilige, gehaltesorg te verseker. Die geregistreerde professionele verpleegkundiges (GPV) se bewustheid en persepsie van rolmodellering as ʼn onderrig- en leermetode hang af van hul bydrae tot groei en ontwikkeling. Rolmodellering is ʼn deurdringende manier waar ontwikkeling deur middel van observasie en oefening in die kliniese omgewing plaasvind. Dus is dit ʼn fondasie in gesondheidsberoepe onderrig. Die doel van die studie is om te bepaal of GPV’s rolmodellering verstaan as ʼn onderrigstrategie. ʼn Kwalitatiewe fenomonologiese ontwerp was gebruik om data in te win deur middel van semi-gestruktureerde onderhoude met ʼn doelbewuste insamelingsmetode van sewe GPVs werksaam in die kritieke sorgeenheid van ʼn tersiere onderrighospitaal in die Weskaap. Onderhoude was op band opgeneem en woord vir woord getranskribeer. Die getranskribeerde data was geanaliseer en gekodeer waarna twee temas geidentifiseer is. Die eerste tema het die deelnemers se persepsie van rolmodellering geillustreer en openbaar dat nuwe verpleegsters spesifieke gedrag doelbewus en onwetend nastreef en naboots en dat hulle kennis en vaardighede in kritieke sorg verpleging aanleer, insluitende “sagte” vaardighede. Verder was refleksie gebruik deur die rolmodel om ʼn verbintenis te maak tussen inligting of optrede wat leer bevorder. Die tweede tema dui daarop dat ʼn aantal uitdagings die GPVs en nuwe verpleegsters verhoed om te leer en onderrig deur middel van rolmodellering. Die bevindinge illustreer dat GPVs nie altyd verstaan dat rolmodellering plaasvind terwyl normale take uitgevoer word nie. Deelnemers se gewilligheid om te onderrig deur middel van rolmodellering kan nie bevraagteken word nie, maar die effektiewe aanwending daarvan moet wel versterk word. Die studie verteenwoordig die eerste van sy soort in die instansie en kan waarde toevoeg deurdat dit die verstaan van rolmodellering as ʼn onderrig- en leermetode vir gesondheidswerkers bevorder.
