Patterns of weight gain prior to the diagnosis of pulmonary tuberculosis (PTB) in a group of 149 children notified as suffering from PTB were studied. Of 50 children with probable PTB, 33 (66 per cent) were either losing weight or failing to gain in weight, of whom only 14 (28 per cent) had a weight < 3rd percentile of weight for age. In the case of the remaining 99 children with suspected PTB 28 (28 per cent) had either lost weight or failed to gain in weight; of those 20 (20 per cent) had a weight < 3rd percentile. In both groups some children with a weight for age < 3rd percentile were gaining constantly in weight parallel to the 3rd percentile and thus although nutritionally growth retarded were not acutely malnourished.
