Abstract
ENGLISH SUMMARY : Every person is entitled to basic health care, but access to health services is a global problem. Many rural communities have no basic health services available, with health professionals concentrated in and around urban areas and in private practice. Registration of health professionals with the Health Professionals Council of South Africa is only possible when the one year of community service has been completed. Compulsory community service commenced in 1998 and was expanded to include all the health professions by 2016. The goals of compulsory community service are to take health services to rural communities, to foster continued education, and to stimulate retention of health workers in rural communities through improved social accountability. This, however, is not always the goal of the health professionals performing this community service. Various studies regarding the perceptions of health practitioners on compulsory community service have been done. This study was intended to explore the perceptions of the health practitioners on their compulsory service in a rural district in the Western Cape. Nine newly qualified professionals who had completed their community service in the Matzikama Sub-District between 2006 and 2012, participated in the qualitative study. The Matzikama district services are delivered at the local provincial hospital, as well as at the 17 clinics that are spread over the entire district. Three participants worked at the hospital and did not attend to any clinics in the district, while six were allocated to the clinics, but not to the hospital. Internal and External factors were identified as themes of the semi-structured interviews held with the study participants as those which influenced their experience of community service in the Matzikama Sub-District. The perceptions of health professionals in this area highlighted a lack of support for newly qualified professionals, as well as a need for management and leadership skills to be better incorporated in their pre-qualification training. Final recommendations to the Department of Health include better orientation and planning regarding the allocation of professionals to rural areas as a mechanism for providing a better experience for the newly-graduated health professional.
AFRIKAANSE OPSOMMING : Elke mens is geregtig op basiese gesondheidsorg, maar die toegang tot gesondheidsdienste is ‘n wereldwye probleem. In baie landelike gemeenskappe ontbreek dit aan basiese gesondheidsdienste, terwyl professionele gesondheidswerkers gekonsentreer is in en rondom stedelike gebiede en in private praktyke. Registrasie by die Raad vir Gesondheidsberoepe in Suid-Afrika is slegs moontlik nadat die verpligte gemeenskapsdiens van een jaar voltooi is. Die diens het in 1998 begin en is geleidelik uitgebrei om uiteindelik teen 2016 alle gesondheidsberoepe in te sluit. Die doel van verpligte gemeenskapsdiens is om die dienste na die landelike gemeenskappe te neem, om as voortgesette onderrig in die praktyk te dien, en om ‘n retensie van gesondheidswerkers in die gemeenskappe te probeer bewerkstellig deur sosiale verantwoordelikheid te stimuleer. Dit is egter nie altyd die doelwit van die gesondheidswerkers wat hierdie diens lewer nie. Verskeie studies is gedoen om die persepsies van gemeenskapspraktisyns oor verpligte gemeenskapsdiens te ondersoek. Met hierdie studie is gepoog om die persepsies van verpligte gemeenskapspraktisyns in ‘n landelike distrik in die Wes-Kaap te ondersoek. Nege van die gemeenskapspraktisyns wat hulle gemeenskapsdiens tussen 2006 en 2012 in die Matzikama Sub-Distrik gedoen het, het aan die kwalitatiewe ondersoek deelgeneem. Gesondheidsdienste in die distrik word gelewer by die plaaslike provinsiale hospitaal, asook by die 17 klinieke wat versprei is oor die hele streek. Drie van die deelnemers het slegs in die hospitaal gewerk, terwyl die ander ses slegs by die gemeenskapsklinieke gewerk het. Sowel interne as eksterne faktore is as temas uit die semi-gestruktureerde onderhoude geïdentifiseer as die faktore wat hulle ervarings van gemeenskapsdiens in die Matzikama Sub-Distrik beinvloed het. Die persepsies van die gesondheidswerkers was dat daar ‘n tekort aan ondersteuning vir pas-gekwalifiseerde gesondheidspersoneel was, en dat daar ‘n behoefte was dat leierskaps- en bestuursvaardighede in die voorgraadse opleiding ingesluit word. Die finale aanbevelings aan die Departement van Gesondheid is dat aandag gegee word aan die orientering en toewysing van gesondheidswerkers na landelike gemeenskapsdiensareas om gemeenskapsdiens ‘n beter ervaring vir pas-gekwalifiseerde gesondheidswerkers te maak.
